How to create and edit a questionnaire for clients to complete
Clients can complete extra questionnaires about their event once they've booked. You can also limit the display of the questionnaire based on booking details such as Event Date or Event Type. Control where the user goes after the form is submitted.
Create / Edit a Form
From your dashboard, select the Bookings icon.
Ensure you are on the Forms View.
To add a new form click +Add Form or to edit an existing form, click on it, or click on the Ellipsis (3 dots) and select Edit and make the necessary changes (follow the steps for guidance).
Enter a Form Name (eg. Customer Feedback) and choose Form Purpose > Capture additional data.
Click Create & Edit Form and your Form will open.
Ensure you are on the Build tab. You can add more fields into your form, using the + button (not the + in the circle if you have not added a logo in your branding yet) and the field chooser will appear.
You can choose any of the job management fields or add your own field. To add your own field, click on the + button and select +Add New Field. A drawer will open.
Enter a Name (eg. "Are you having a First Dance? )
In Type, choose from the dropdown menu. Your question choice will determine which Type you select. (eg. if you were to ask "Are you having a First Dance? your Type would be "List"
In List Settings, tick the checkbox for Add to core Job fields? if you wanted to have this question added into the Job Fields Management.
In List Settings, click on +Add, you are able to add an answer. Using our example, you would have Yes in one box, and +Add to have another option and enter No.
You can make one of the answers a Default by Ticking the checkbox
You can give each answer a Color by clicking on the Color box for that option and choose a Color.
Click Save
You are able change the order of the chosen fields by using the Move Cursor on the left hand side and move them to your chosen order.
Click Save
Configure A Form
On the Configure tab, in After Form Submit, you can choose from the following :-
Thank You Page
Client Portal
Voice
You have the option (by clicking +Add Action) to redirect to
Sign Contract
Payment
Set Status to
Default Tax Display will show your default choice, or choose from the following :-
Included
Excluded Hidden
Excluded Monetary
Excluded Simple
Thank You Page - allows you to edit your Thank You Page Message.
Click Save.
